Ubuntu Dementia facilitates learning about dementia and person-centered living through:
  • Workshops/Conference Presentations
  • “In-house” training for all people who work in nursing homes, assisted living, adult day, etc.
  • Training on CMS’ Hand in Hand training toolkit
    • We offer Train the Trainer programs to help you facilitate Hand in Hand in your organization
    • We will facilitate the Hand in Hand training program for you in your organization

Gathering Information
Gathering information about how life is for the people who live and work in your community is an essential. foundational phase in a person-centered journey.


An organizational assessment is objective appraisal of where your organization is in providing person-centered dementia care, as well as where you would like to be and what you need to get there. As outside consultants we bring to you a different set of eyes, our professional experience and knowledge in dementia, and what we have learned from our work in communities serving people with dementia across the country.

An organizational assessment provides you with the information and insight you need to become a better place for people with dementia to live as well as a better place for team members (staff) to work. Building a community where people live well is not only "the right thing to do", it will distinguish you as a leader in good care.

The organizational assessment process also better prepares you for an increased focus on dementia care by surveyors (per CMS letter S&C: 13-35-NH) and the dementia care focused survey (per CMS letter S&C: 14-22-NH).

There are three types of organizational assessments we provide:

1. Pre-Training Information Gathering

Prior to staff training we will spend a half day at your community to learn how things are going. From this we will develop a customized training program to meet your needs.

2. Full Information Gathering

A thorough organizational assessment process gathers information about your community from residents, families, team members (staff), clinicians, leadership and other stakeholders as identified. This includes what goes well at in terms of dementia care, what are areas of opportunities and improvement, what person-centered activities are currently happening, and all stakeholders’ vision for a person-centered community.

This process involves:
  • Surveys to team members, resident, and family members
  • Onsite focus groups conducted by two consultants with team members, residents, family members, leaders, volunteers, associated clinicians, associated services providers, and other stakeholders;
  • Onsite “observing” by two consultants; and
  • Formal assessment using existing person-centered measurement tools/checklists (evaluating the overall “person-centeredness” of a community).
  • We would organize information, analyze for themes, and present this back to the leadership team with recommendations. This would include the development of materials that leadership can share with the team on the findings of the appraisal, such as PowerPoint, written materials, etc.

3. Mystery Shopping

We also provide "Mystery Shopping" in which we will pose as a potential family member of a potential resident/client. Based on our experience we will provide you a report, with identification of strengths as well as areas for improvement.



Bringing It To Life
We provide general consulting and coaching to bring person-centered values and knowledge to life. We will get you where you need to go and help you make the changes you need to make so that people with dementia can live well.

This includes helping you:
  • Operationalize recommendations from an organizational assessment.
  • Develop action plans
  • Facilitate team meetings

We will teach you how to fish!

All of our consulting services come from a new vision of dementia, and how we care for people living with dementia, in a way that helps them live well based on these values.
